MPHOA has a number of documents and forms to simplify tasks and/or help us manage the Association better, including:
Dues Payment via Bookkeeper's Web Portal - This document provides instructions for how to pay your dues automatically by visiting N.A. Shade's web portal, creating an account, then choosing whether to pay via eCheck or credit card. Once you select a payment method, you can choose whether to make a single payment or recurring payments. Note that paying your dues via the web portal is an alternative to paying your dues via your bank's bill pay, so if you already pay your dues via bill pay, you don't have to change anything.
Architectural Change Application - Download and complete an ACA form when you plan to do a project that will be visible from outside your unit. See Architectural Guidelines on Rules webpage for information about when it is required.
Key, Parking Tag, Entry Gate Remote order form. Note that Affirmative Management does not have front door keys or mailbox keys. If you lose your front door key, call a locksmith. If you lose your mailbox key or you didn't receive one when you moved in, read this memo. If the advice in the memo doesn't produce results, you can visit the Mission Station branch at 1050 Kiely or call them at 408-243-0128 and ask for assistance.
Resident Registration form - New residents: please complete a RRF form and send it to Affirmative Management so they can reach you if there is a problem in the complex that may affect you. The same form can be used to update how your name appears on the display in the silver intercom boxES at the Halford Avenue & Miramar Way entrances.
Water Shutoff Notice - Visit the Plumbing Projects, Leaks webpage, then download, fill out, and post a WSN on all units in your building when you are doing a plumbing project that will require water to the entire building to be shut off.
