MPHOA has a number of forms to simplify certain tasks or help us manage the Association better, including:
Automatic Dues Payment form - This ACH form will enable you to pay your dues automatically. There is no easier way to pay your HOA dues because you never have to write a check. Plus, ACH automatically adjusts when dues change; your bank's bill pay doesn't do that.
Architectural Change Application - Download and complete an ACA form when you plan to do a project that will be visible from outside your unit. See Architectural Guidelines on Rules webpage for information about when it is required.
Key, Parking Tag, Entry Gate Remote order form. Note that Affirmative Management does not have front door keys or mailbox keys. If you lose your front door key, call a locksmith. If you lose your mailbox key or you didn't receive one when you moved in, read this memo. If the advice in the memo doesn't produce results, you can visit the Mission Station branch at 1050 Kiely or call them at 408-243-0128 and ask for assistance.
Resident Registration form - New residents: please complete a RRF form and send it to Affirmative Management so they can reach you if there is a problem in the complex that may affect you. The same form can be used to update how your name appears on the display in the silver intercom boxES at the Halford Avenue & Miramar Way entrances.
Water Shutoff Notice - Visit the Plumbing Projects, Leaks webpage, then download, fill out, and post a WSN on all units in your building when you are doing a plumbing project that will require water to the entire building to be shut off.
